Accidental Conversion of Roth 401k to a traditional IRA. How to correct?

In 2016 I decided to transfer my Roth 401k with a previous employer to a Roth IRA. The investment advisor made a mistake and checked the box it was a conversion to a traditional IRA. How do I correct this error?



What does your 2016 1099R and 5498 indicate? Error should have been discovered when filing the 2016 tax return, if not sooner.  A Roth is not eligible to be rolled into a TIRA, but if it happens it may be impossible to undo all the damage. Of course, with enough documentation you would have a cause of action against the advisor and or the IRA custodians, depending on how the transfer was done.
